Directions from Orlando, FL to CCS Medical Clearwater, FL

• Take I-4 West towards Tampa.
• Follow signs for I-275 South to SR 60 West.
• Follow signs for SR-60 West towards Clearwater.
• Merge onto N. Memorial Hwy/FL-60 W/FL-589 N.
• Take the SR-60 W exit toward Clearwater.
• Merge over into one of the three left hand lanes - Keep left at the fork in
  the road.
• Merge onto FL-60 West (also called SR-60 - Courtney Campbell Causeway)
  The Causeway Bridge is about 9 miles long.
• After traveling over the Causeway, at the 2nd Traffic Light, take a Left (South) onto CR-611 S. (McMullen Booth Road) over the Bayside Bridge.
• After the Bridge, continue straight onto 49th ST. N.
• After the High School on your left, you will see a glass building on the left – CCS Medical.
• Either cross traffic on the left and turn into driveway, or go to 140th Ave. N and make a left U-TURN at traffic light.
• CCS Medical (Chronic Care Solutions) will be on the right – large glass building.
• Main entrance is at the long, green awning - Building 1.

• Follow signs for Airport Exit.
• Merge onto North Memorial HWY/FL-60 W/FL-589 N.
• Take the SR-60 West exit toward Clearwater.
• Merge over into one of the three left hand lanes - Keep left at the fork in
  the ramp.
• Merge onto FL-60 West (also called SR-60 - Courtney Campbell Causeway)
  The Causeway Bridge is about 9 miles long.
• After traveling over the Causeway, at the 2nd traffic light, take a left (South) onto CR-611 South. (McMullen Booth Road) over the Bayside Bridge.
• After the Bridge, continue straight onto 49th ST. N.
